This is the application to use if you are applying for surplus designs included in an earlier application filed with a form DF2A. Refer to DF2A and DF2B notes for guidance on how to complete the form, and also the How to apply to register a design booklet with the changes in application procedure.
Send us your application
Please send your application to our address below. You must include your filled in DF2A and fee sheet forms, one copy of the illustrations of your design, and your fee.
